Clinical Observation on Gastroduodenal Ulcer


Abstract
113 cases of gastroduodenal ulcer treated surgically at St. Paul¢¥s hospital from Jan. 1968 to Dec. 1972, were studied.
The following resuls were obtained.
1) The highest incidence for indication for operation in gastroduodenal ulcer was perforation (67.2%), and intractability (20.4%), stenosis (6. 2%) and bleeding (6.2%) were followed. 2) The ratio of malignant and benign ulcer was 14.2 : 85.8.
3) The ratio of ulcer between stomach and duodenum was 47. 8 52.2:
4) In age distribution, the highest incidence was 5th decade; and 4th decade was followed. The sex ratio between male and female was 7 : 1.
5) Seasonal occurrence of gastroduodenal ulcer was predominant in autumn and spring.
6) The duration of symptom was various, but the highest incidence was between I and 3 years (24. 8%).
7) Analysis of clinical symptoms and signs revealed in order of abdominal pain(90.2%), abdominal
rigidity (67%), nausea, vomiting (65%), abdominal distension (48%) etc..
8) The findings of free air on abdominal film(chest film) in ulcer perforation was 63 cases(82. 8%). 9) Of the 113 cases, 38cases (33. 7%) belong to blood group B. 10) Leucocytosis ranging above 10, 000/mm3 was noted in 47% and hemoconcentration revealed
23%.
11) In the location of gastroduodenal ulcer, 108 cases (95.7%) was on the anterior wall and 5 cases (4. 3%) was on the posterior wall. The cases of below 0. 5 cm in diameter of perforated ulcer was 54 (71.2%) cases.
12) In operation procedure, subtotal gastrectomy was performed in 101 cases (89. 4%), and simple closure was followed in 8 cases (7.08%).
13) Postoperative complication revealed in 32 cases(28.2%), of which infection was most frequent. The over all mortality of gastroduodenal ulcer was 1. 78% or 2 cases.
